Unfortunately, the Jensen is balanced to unbalanced only.  IMO, if you're going the other way, an active solution is the best bet.  Symetrix used to make a nice one that was resonably priced and quiet but I think they've discontinued it now.

True, the one I linked is opposite from what was requested (sorry, late night). However, Jensen is pretty flexible and can probably make one at the required input/output. If not you can always build your own with a pair of Jensen JT-11P-1 transformers. A quick project but note that the Jensen transformers are not cheap. They are superb in performance however.

There are less expensive options, though lacking personal experience, I can't opine as to the quality.

http://www.edcorusa.com/products/matchers/index.html
and www.cinemag.biz also makes high quality balancing transformers

As stated, Jensen is a great source (though not inexpensive). The actual product depends on whether your need is for an output or input transformer. For example, I use one of their output transformers to convert the unbalanced output of my CD player to balanced for connection to a balanced input on my preamp.

In addition to others mentioned (i was going to mention cinemag and edcor but Occam beat me to it) there are:

Sowter

Lundahl 1676 (pricey)
Tribute custom made ones (pricey but suppose the ultimate...I just bought a pair recently but haven't tried them yet)
NOS iron off of ebay, such as UTC and the like (unknown quality)

Lots of options really...

Uh........I assume that this is on the "receive" side of things. A lot more options this way.

Depending on what transformer you pick, most seem to work better in a 4:1 step-down configuration. Trust me on this one........

You might also consider the EBTech 2-Channel Hum Eliminator in your decision. It is ready-made (not DIY as you requested), and is a surprisingly good product, especially for the application you are describing. You might need to use adapters, as it uses 1/4" TRS smart-jacks for inputs/outputs. :)

I have used the Ebtech before and it is good (just HATED the 1/4" jacks connectors) - But I was thinking of getting just the transformers so I can insert them into the dac - and use less cables and connections.

Alternatively, you could just use rca to xlr adapters from Neutrik or wire a cable with the connectors wired appropriately. Courtesy of Wayne of diycable -

Not a fan of that- I've used them in the past- wasn't happy- I might as well change the connector on my XLR cable to RCA than make an adapter.
