Is the Bryston 4b st a fully differentially balanced amp?

No it is not. The loudspeaker is floating between one amplifier and ground.

Only the 7BSST and 14BSST (and probably the new 28BSST :D) are really differential amps. There the loudspeaker float between a plus and a minus amp. Doesn't have to say that a differential design is automaticly better.

There is one exception, and that is if you order the 7BSST in Parallel mode. Then the two amplifers are paralleld and the speaker is float between them and ground.
