Hi Jim,
It was working until this afternoon. I could not get any output from my above. The LED still blinks, ie. as it was working, ie. slow pulses then faster and faster. I only get a hum from the MC output. I checked the DCoutput from my AC adapter and it is 12.18v DC but after resistor R33 it is 12.17v but after R35, it is only 10.94v. Is the low voltage the cause of it not working?

Sounds like it is working properly.  I use some voltage dropping RC filters in the various supplies to remove hum.  Are the MM outputs ok? What about normal line level out?  What is gain (amplitude) set to?
