This is a custom build PVA pre amplifier built by Electra Print. A base PVA has two inputs, both for solid state devices and a single set of output jacks.
This one has 3 sets of inputs optimized for solid state, and 3 sets of inputs optimized for tube sources. Has two sets of outputs, one pair being full range, and the other pair with a 6dB 100 hertz cut off meant for a subwoofer output. The case is built from a composite wood material with a hammertone finish. This means there is only one ground plane on the pre-amp, and there are no loops or interfence from the chassis. Yes its a plain Jane look, but its a mighty nice pre amp.  Sorry the flash on my camera makes it look like there are major flaws in the finish, its not that way.

The inputs were never silkscreened with numbers, so I have some temporary indicators on it to show the matching position to the inputs.

The PVA is a great pre-amp, as the nickel core transformers provide impedance buffering from the source component and the amplifier, and it does provide a fair amount of gain due to the transformers I once had a Jeff Rowland Consummate pre-amp here, and that was a $5200 pre amp in 2001, and it sounded no better than the PVA.

So if you want a quality passive pre-amp for an affordable price, ths is it. You won't get better without spending some real coin.
