'Partial' dual op amp failure.

'Partial' dual op amp failure.

G'day all, I wonder how common this kind of op amp failure is?  I was testing my DIY ESP P06 phono stage tonight channel by channel and noticed that whilst both channels appeared to work normally it was apparent that at full amplifier volume (but not playing a record), the left channel was quite 'hissy' compared to the other right channel.  I swapped the LM4562 with another new one and all hiss disappeared! 

Curious, obviously the original LM4562 was partially faulty!  Has anybody struck this mode of partial failure in their own work?  Regards, Felix.

The manufacturers call it ''altered'' they not consider it damaged.
Obviously this part was not tested by the equip builder.
Its needs a analog meter to pickup this fault not a digital meter.
Transistors also suffer from this illness.
