I lost six of twelve new production with break-in and screening from a reputable tube vendor. These were used in a phono stage and line stage. Failures in each, starting at 100 hours and continuing to 400 hours with a seventh sounding like it will fail soon. Both pieces of gear run the tubes conservatively. I measured Vp on all triodes on both components to make sure nothing had gone south and was putting undue strain on the tubes.

It's too bad. A nice sounding tube but at $45 a pop, a very bad investment.
