Looking for Pre-Amp/DAC With SPDIF/COAX/USB

....and if it could do DTS/DD that would be great also :)

Any suggestions?

I would get an AV Receiver with preouts. The older Harman models have quite decent DAC sections build in wich can compete with the DAC´s in your pricerange you´re looking for imo. They´re well known for t´s stereo performance, at least over here in germany. Even if you don´t use the rear channels you get a powerful remote controlable stereo amp / preamp and DTS / AC3 capable DAC for cheap.

I'm looking for a cheap and cheerful DAC that runs on 220v and can take the optical from my PS3 and output balanced analogue XLR? (I know not exactly typically cheap) :)

I got one of these^^^ love it..I use a PS3 toslink optical output to the DAC to an integrated amp, not sure if it fits your power requirements though..check it out..FWIW

Neither the Maverick D1 or the Grant Fidelity DAC-09 have balanced XLR outputs.
 
The Cambridge Audio DacMagic has balanced XLR outputs and uses an external 12VAC @ 1.5A power supply.  The unit should be sold with a suitable 220V input supply in your country or a proper replacement 220VAC input supply could be sourced locally.  The DacMagic retails for $429 US and can be found for ~ $300 US or less used.
 
The Citypulse DAC DA7.2x II has balanced XLR outputs, 115V/230V dual voltage IEC input and remote control, and sells for $425 US.

I've got the matrix Mini dac.  Sounds great,,has volume control and XLR outs. see link
