Do you think the performers are aware of how bad the sound is sometimes?

I am not sure... I think that if artists would be truly aware how their performance sounds, the good engineers would be paid more and be in much higher demand.
Many sound engineers are hired for a (part of the) tour, they only do about 20 shows and then are gone again.
I remember when Joe Bonamassa was here doing rehearsals for his tour he was at the front of house area with the sound engineer going over all details on how he wanted things to sound live. Not many artists do this, I have only seen this happen a few times in all my years in music.
It is impossible for them to actually hear what the audience hears, a room sounds very different when filled with people. As such they always have to rely on someone elses opinion.
I think that "the sound" should not be determined by just one person, there should always be someone with a second opinion. Sound guys go deaf too...
