How should this be set up if running balanced from a Bryston pre amp?  What is the determining factor?

Its just a gain switch, try both and decide which one you like.

There is no hard rule here. Try and listen for a while, and then go with the one you like best.
I have mine set to 26 dB because I have the feeling that the sound is slightly more dynamic then.

When in doubt, follow the manual  It says 1V for unbalanced and 2V for balanced.

My I/C are all in Balance XLR but I had it at 1V or -29db which according to the manual its recommended for Single Ended not balance. I listened at this setting/gain level for about 2 weeks and realized I can't really push the volume beyond 9 O'clock for long listening sessions.

I switched it to the recommended 2V or -23db setting for balance and find it suits me better. I can now push the volume above 9 O'clock and it sounds much more BALANCE   :drums:
