Headroom Micro Amp. This is the earlier model that can run on either two 9 volt batteries (not included) or the included brick power supply. I bought it as part of a Headroom package (with the Desktop PS and Micro DAC) from a Head Fi-er last winter and I have the later model, so I don't need this one. The difference in sound quality between the two is very small however and the latter doesn't have the battery/portability capability of this unit (which I don't need).
$75 including Paypal, plus shipping. Prefer to use Priority Mail, and it will fit into a medium flat rate box, so it'll be $13.
