Sony PS1 model SCPH-1001.  I purchased this unit thinking I would mod it and use it in my 2 channel system.  I have had it for a few months and have only used it a couple of times, I typically go ahead and rip new CDs for use with my Squeezebox.  The unit works fine, the lid release sometimes sticks but releases when pushed again, this is easily repaired.  It comes with a power cord and single controller.  There are many websites that talk about how good this unit can be once it is modified.  I don't have the original box but will pack it well for shipping.  Please PM me with questions.
