www.equitech.com. Purchased new in February of 2010. I have fitted a 2 pole common trip circuit breaker and a single socket for the output. I have also fitted a short piece of Romex with a plug for the input. No operational issues. A few minor scuffs and scratches. Will ship with Equi=Tech supplied paperwork.
