It is a Niteshade Amplifier, made several years ago.
The early models did not have variable feedback and it could possibly use a few circuit updates. Several changes/revisions have been made in the last several years.
The power transformer looks like a Fisher and the outputs from an organ amplifier, such as a Thomas. The middle tube is most likely something like a 0D3 regulator.
