The test was created by James E. Galvin and New York University Langone Medical Center.
QUESTIONS AND SCORES OF INSANITY FAST SYSTEM ASSESSMENT:
The test measure changes in cognitive and functional abilities of the patient. You should compare the patient now as it used to be - the central issue is change. In each category, choose the phrase that best describes the patient and write down how many points it is worth. Add up the points for each question and see, at the end of testing, what that score means.
Not all features need to be present for the answer to be chosen.
SCORE:
The test is not equivalent to a medical diagnosis. The final score ranges from zero to 30, and higher scores suggest greater cognitive loss. The standards of evaluation from the test in 267 patients indicate that:
Normal: points 0-1
Mild cognitive impairment: 2-5 points
Mild dementia: 6-12 points
Moderate dementia: 13-20 points
Severe dementia: 20-30 points
High scores indicate that the patient must undergo a medical evaluation for a formal diagnosis. Scores "normal" suggest that it is unlikely that the patient suffering from dementia, but it is also possible that the disease is in very early stages. If there is suspicion of dementia for other reasons, it is good to seek professional help.

MEMORY
0 points - No loss of obvious memory. Irregular forgetfulness that do not interfere with daily activities.
0.5 points - mild forgetfulness and regular or part of events that can interfere with daily activities; repeats questions and sentences, places objects in unusual places; forgets appointments.
1 point - slight to moderate memory loss, most noticeable when dealing with recent events; interferes with daily activities.
2 points - Loss of moderate to severe memory; new information is quickly forgotten; just remember information learned with great effort.
3 points - severe memory loss; almost impossible to remember new information; Long-term memory can be affected.

ORIENTATION
0 point - Fully oriented as the people, space and time practically forever.
0.5 point - Take a hard time keeping track of the time; can forget dates more frequently than in the past.
1 point - mild to moderate difficulty in tracking the time and sequences of events; forget the month of the year; oriented in familiar places, but gets confused outside of known space; is lost and is wandering.
2 points - Moderate difficulty to severe; usually disoriented as to time and space (relative or not); often has difficulty remembering the past.
3 points - Guided only by one name, although it can recognize relatives.

DECISION MAKING AND PROBLEM SOLVING
0 - Solves everyday problems without difficulty; copes well with personal and financial issues; making skills of consistent decisions in its history.
0.5 - Take debility (or greater delay) in problem solving; difficulty with abstract concepts; decisions still consistent.
1 - moderate difficulties in dealing with problems and make decisions; delegate many decisions to third parties; perception and social behavior may be slightly compromised; loss of discernment.
2 - Severely impaired in handling problems, taking just simple personal decisions; perception and often debilitated social behavior; undiscerning.
3 - Unable to make decisions or solve problems; the 3rd parties take almost every decision for him or her.

ACTIVITIES AWAY FROM HOME
0 - takes on their profession independently, performs acquisition, community and religious activities, volunteer and social groups.
0.5 - Take debility in these activities compared to previous performances; slight change in skills as a driver; still able to handle emergency situations.
1 - Unable to function independently, but still able to keep up with social engagements; It seems "normal" to others; noticeable changes in skills as a driver; concerns about her ability to handle emergencies.
2 - No ability to practice outside activities independently; It seems well enough to be taken for outdoor activities, but generally need to be accompanied.
3 - Unable to perform activities independently; It seems too ill to be taken to activities outside the home.

0.5 - Mild debility or loss of interest in these activities; difficulty in operating equipment (mainly younger).
1 - Mild but definite weakness at home and hobbies; abandoned task more difficult and more complicated hobbies and interests.
2 - Preserved only the simplest daily activities; very limited interest in hobbies, met with little rigor.
3 - No significant ability in housekeeping or in previous hobbies.

HABITS FOR PERSONAL HYGIENE
0 - Fully able to take care, dressing, washing, bathing, using the toilet.
0.5 - Changes in light skills with these activities
1 - Need to be reminded to go to the bathroom, but you can do it independently.
2 - Need help getting dressed and clean; occasionally incontinent.
3 - Requires considerable assistance with hygiene and personal care; frequent incontinence.

0 - No difficulty of language or words of oblivion; reads and writes as well as in the past.
0.5 - Mild difficulty but shows consistency in finding the words or descriptive terms; It may take longer to complete reasoning; light comprehension problems; poor conversation; there may be effects on reading and writing.
1 - Moderate difficulty in finding the right words; unable to name objects; noticeable reduction in vocabulary; listening, speaking, reading and writing reduced.
2 - Moderate Weaknesses or severe in speaking or understanding; difficulty communicating thoughts to others; limited in reading skill and writing.
3 - Severe deficits in language and communication; little or no speaks understandable.

0 - Normal Attention, concentration and interaction with the environment that surrounds it.
0.5 - Light of attention problems, concentration or interaction with the environment; may seem sleepy during the day.
1 - moderate problems of attention and concentration; can stay staring at a point in space or closed eye for several periods; increasing daytime sleepiness.
2 - Spend considerable part of the day sleeping; do not pay attention to your surroundings; when you talk say things illogical or unrelated to the topic.
3 - limited or no ability to pay attention to the external environment.

Are we supposed to answer these ourselves? It doesn't seem very thorough without some kind of clinical study, but I guess it is free. I know a few people that have been below 0 on many of these, even in their teens. Boy are they in trouble.

My mom was under medical care for early memory loss (a better, more encompassing term than Alzheimers that can only be confirmed via brain biopsy at time of death).  Part of her assessment was a series of questions like: Do you know where you're at?  Do you know what city you're in?  Do you know why you're here?  What year is it?  What month is it?  What day of the week is it?  What season is it?  Fortunately she passed from another cause before the disease progressed much further.

She had good days and bad (she scored very well the first time we had her tested and poorly the next time).  Staring at her only grand daughter and not being able to recall her name was the point I stopped denying she was simply "pleasantly forgetful".  IMO memory loss among the elderly is over diagnosed.

Memory loss can take various forms.  Many become angry or mean spirited (partially relating to their frustrations with the disease).  Some become afraid of the dark or otherwise mix up their day/night sleeping patterns.  Some become mindless busybodies, wandering and messing with everything within reach.  So they're prone to fall down stairs, turn on stoves, and grope everything/everyone.  And some strive to elope.  (I took a class in elopement.  They tend to wander towards the sun, downhill, and follow some sort of pathway - even walking into rivers.)

Staying active physically and mentally is the best way to delay the onset.  Writing, longer documents the better, is one of the best exercises for your mind.

Docs also use clinical tests and use this same questions to the patient or family.
