last one for now anyway.

I have some nice GP phono jacks on hand so I didn't order any from the parts list.  The ones I have give me the option of using the included insulating washers or not using them.  Should the jacks be connected directly to the chassis box for grounding or should I use the insulating washers?

Depends where you put them.  Safer (against hum) if insulated and running PCB ground to grounding jack.
