I would only real requirement is optical and coax inputs, I suppose USB might be helpful, and firewire might be fun , but only optical and coax really needed. Any suggestions? Thanks.

I know of two.  I believe the Benchmark dac includes a linestage pre.  Also Monarchy has a dac/pre model NM24.  Both have been very favorably reviewed.

I don't have a list but one of the best "bang-for-the-buck" units I've come across is the Tube Audio Design TADAC.  Some have recently sold in the classified section here (including mine - I chose to move to separates).  I haven't heard many of the different products but Paul at Tube Audio Design seems to make good products for the budget-minded, and he stands behind them with very responsive service.
